COVID-19 On-Campus Student (age 12 and up) Booster Shot Clinic

We are thrilled to be able to offer a COVID-19 Booster Shot Clinic for students from 2:30 -5:30 pm on Senior Patio.

Required Documents: 

  • Completed consent form 
  • Consent form is available on appointment link.  Answer questions online, download, print and bring to appointment. 
  • Extra consent forms will be available on-site
  • A photocopy of Medical and/or prescription insurance card if applicable. Noninsured individuals are welcome.

Appointment confirmation: 

  • Confirmation notification will be sent via email or text.  Email will come from Albertsons Patient Care.
